We welcome new patients who wish to join us who live within our practice boundaries. Our patients live predominently in Chelston and Livermead although the practice boundary includes all of Torquay & the fringes of Preston to Preston Down Road.

As you will see from the website, we are a small, friendly practice & we take pride in our ability to be approachable & to provide friendly service to all family members.

We want you to feel relaxed about joining us and to make your registration as simple and straightforward as possible.

Firstly, you can simply come to see us at Old Mill Surgery during our open hours. We will provide you with the forms you need to complete & we will help with any initial concerns you may have about choosing us as your family surgery. Please bring photographic proof of identity (Ideally a Driving Licence or Passport) - this is important as we cannot proceed without a document of this kind. If this is not available, we can consider other forms of ID such as a Utility bill showing your name and address, a Student ID card, Works Card etc.

Secondly, if you prefer, you can use these printable forms;  Surgery Registration Form, GP Registration Form, Ethnic Origin Form, and finally First Language Form

• Complete them by hand.
• Please do not attempt to send these to us online.
• Sign the forms as indicated.

Come and see us bringing your 4 completed forms (one set for each member of the family) and the proof of identity, as mentioned above.

After this, part of our welcome will be for the doctor to meet you and to talk to you in an informal consultation (we call this a "New Patient Registration check"). This will be 20 minutes for adults and 10 minutes for children over 5 years (younger children are first seen by the health visitor) and can be arranged from 24 hours after receiving your forms. Please telephone or call in to make your appointment and mention that it is a new patient registration.
